Look for a larger single-room restroom labeled "multipurpose toilet" (tamokuteki toire) or "toilet for everyone" — these are common in stations, shopping facilities, and tourist sites, and often combine wheelchair access, a diaper-changing table, and ostomy equipment in one room.
Checking a map app or accessible-toilet-finder app for the nearest one ahead of time can save scrambling on the day.
These rooms tend to get used by general visitors too when things are crowded, so there's an ongoing awareness campaign asking people to prioritize those who actually need them.
